2025 - 2026 Platinum IE Coaches Contract and Substitute W9Contract Date is from April 1, 2025, to March 31, 2026.
This confidential form allows Platinum Inland Empire to collect information from non-employees, such as freelancers or contractors, who earn compensation from our company. It also enables you to accept/refuse compensation and/or designate portions of your compensation. This document also defines the contract terms between you, as a non-employee, and us, Platinum Inland Empire.

4 Exemptions (codes apply only to certain entities, not individuals; see instructions)
8. Taxpayer Identification Number (TIN)Enter your TIN in the appropriate box. The TIN provided must match the name given on line 1 to avoid backup withholding. For individuals, this is generally your social security number (SSN). However, for a resident alien, sole proprietor, or disregarded entity, see the instructions for Part I, later. For other entities, it is your employer identification number (EIN). If you do not have a number, see How to get a TIN, later.
Under penalties of perjury, I certify that:1. The number shown on this form is my correct taxpayer identification number (or I am waiting for a number to be issued to me); and2. I am not subject to backup withholding because (a) I am exempt from backup withholding, or (b) I have not been notified by the Internal Revenue Service (IRS) that I am subject to backup withholding as a result of a failure to report all interest or dividends, or (c) the IRS has notified me that I am no longer subject to backup withholding; and3. I am a U.S. citizen or other U.S. person (defined in instructions); and4. The FATCA code(s) entered on this form (if any) indicating that I am exempt from FATCA reporting is correct.

Per Player Maximum (Platinum Inland Empire mandates that coach pay must not exceed the set maximum number of paying players. This is determined by age group.
Compensation SummaryA, I accept the maximum monthly compensation in the sum of
from this team for this fiscal year as negotiated with team management within Platinum Inland Empire. Pay is due me monthly by the 5th of the following month. Exceptions: 1. Platinum IE Coach /Assistant Coach/Trainer will be notified if the teams account has non-sufficient funds to pay the coach. 2. Unless previously requested, the team manager is responsible for requesting coach pay each month via the payment request portal. 3. Unless otherwise agreed upon and stated in this contract as additional terms or conditions, pay is based on PAYING players. If a player is past due in fees, the coach will not receive compensation for that player. B. 100% of stipend will be paid to me unless I indicate other instructions below. Please distribute my fee in this manner:
Section B. Scope of Responsibility for Platinum IE Coaches:As a coach for Platinum IE, I agree to be responsible for all aspects of my individual team during the season and understand that I shall:Notify parents/players that open tryouts are mandatory for all players to attend.Players must tryout in their birth year age group.If two (2) teams in the same age exist, players will be eligible to be selected by either team. The Technical Director will have the final decision If teams are coach by two different coaches and both are interest it in the same player.1- Select a team roster at tryouts, notify players who try out, and contact new tryout request within 48 of tryouts.2- Follow the mandated tryout protocol. Any new/unregistered players may only tryout for a maximum of two weeks. If no decision has been made to register the player, a formal written request must be submitted to club directors about an extended tryout for approval.3- Ensure that teams have sufficient number of players committed to training, league matches and agree to tournaments.4- Ensure that your players are in proper Platinum IE approved practice gear. This includes the royal color Platinum IE shirt, black Nike shorts, and black Nike socks. Also, ensure that all players are wearing shin guards during all practice sessions for safety. It’s your responsibility to make sure all players comply with this requirement.5- Coordinate with club directors and facility manager to ensure proper training field location and time.6-Coordinate with club directors on setting appropriate tournament schedule for the upcoming season.7- Coordinate and plan all team communication with Team Administrator/Manager. Communication includes training schedules, team costs, travel costs, coaching expectations, player/parent expectations, etc.8- Provide players with two written evaluations per year, and keep written copy for records.9- Attend all listed Platinum IE specialized trainings, events, functions below.● Coaches’ Meetings● Club Tryouts● College Awards Night● College Info Night● Club Night● P.A.C.E. Awards Night● Other events explicitly stated via coach’s meeting or club email
10- As Coach you must wear approved Nike/Platinum IE apparel and gear at all training sessions, matches and Platinum IE events listed below.● Training: Provided Nike Platinum gear● Match day: Provided Nike Platinum gear.* If not wearing approved Platinum IE training/match gear during practices/games, any BOD member and Technical Director can give you a verbal warning. If you received two (2) verbal warnings within a month, 10% of your monthly pay will be donated to your team.
11- Be fully responsible for game management. This includes in-game decisions, parent-sideline etiquette, any disciplinary action if necessary, and any other situation that may arise that includes the coach, player(s), parent(s) etc. In addition, coach will be responsible for understanding all competition protocols, rules, etc. (i.e. CalSouth, CSL, CRL, US Club Soccer, etc.)
12- At all times while representing Platinum IE, positive character, integrity, and professionalism is mandated.
13- Be responsible for providing equipment and pay for all of licensing costs and other costs.Coaching Requirements:● Coaching License and LiveScan● CPR & First Aid● Any Cal South Mandated Trainings relevant during this contract period
14- Multiple Teams Protocol:● Must be approved by DOC● If you coach two (2) teams, the coach must secure a licensed paid assistant coach and must be assigned to a team. The head coach must provide the licensed paid assistant coach with game management information (i.e. rosters, formations, system of play, etc.) a minimum of 24 hours before start of training/matches. Both the head coach and the paid assistant coach must have a clear understanding of competition protocols, rules, etc. before being eligible to coach (i.e. CalSouth, CSL, CRL, US Club Soccer, etc.)● Coach is responsible for contacting the tournament director when entering two (2) teams in the same tournament to avoid coaching conflicts.By clicking this box, I hereby certify that I have read and completely understand the Scope of Responsibility (Section B) as a coach for Platinum IE and agree to follow all tasks listed above. I agreeAs an non-employee/independent contractor, I understand I am responsible for reporting and law requires paying all appropriate State and Federal taxes on said income. I also agree to release, indemnify, and hold harmless Platinum IE, and its officers, employees, and members for any claims arising out of this agreement including but not limited to claims by taxing authorities.
As a non-employee/contractor I understand that I am not entitled to workers’ compensation insurance or benefits in connection with the team coaching services performed under this agreement. Furthermore, I understand that I can resign as a coach by providing 30 days written notice and that Platinum IE has the right to terminate my status as a coach either immediately (if such cause is necessary due to seriousness of any violation) or with up to a 30 days written notice, whichever is deemed appropriate by the Board of Directors. *
I will not accept any form of compensation outside of the above listed accepted compensation for services being provided by Platinum IE*I understand that I may not accept cash as compensation or reimbursement; all fees are to be paid through the club by the club administrator through the club bank account/s.
I understand that if found in violation, I will risk losing my contract as Coach/Assistant Coach/Trainer for Platinum IE.
